What to have ready.
You do not need to have everything organized before you call. We will guide the conversation. But if you have these things nearby, we can move quickly:
Patient's name and date of birth
We will use this to check Medicare eligibility and begin the admission record.
Primary diagnosis
The illness that is driving the decline — not necessarily every condition. If you are not sure, tell us what the doctor has said.
Attending physician's name
We will contact the physician directly to coordinate the evaluation and certification.
Current location
Home address, assisted living community, or hospital — wherever the patient is now.
