Step 2
We evaluate
An Azalea nurse evaluates the patient — at the bedside before discharge when timing is tight, or at home. We confirm eligibility and complete the Medicare election.
Step 3
We take over symptom care
Once admitted, the patient gets 24/7 on-call coverage, medications, equipment, and the full team. We keep your case managers updated on status.
