For discharge planners & case managers
A hospice partner for UT Health Tyler
UT Health Tyler is the largest hospital in East Texas, and its case managers move a high volume of complex discharges every week. Azalea Hospice is built to be the partner that picks up on the first call, sees the patient before discharge when needed, and takes the symptom-management burden off your floor.
Why teams refer to Azalea
Built to make your discharge easier.
Azalea's primary service hub is Tyler — same-day evaluations for UT Health Tyler discharges are routine, including evenings and weekends.
- Same-day or next-day patient evaluation, including bedside assessments before discharge
- A direct line to an intake nurse — not a regional call center
- We handle the Medicare hospice election paperwork so it doesn't land on your desk
- Coordination with the attending physician and the hospitalist on the plan of care
- Clear communication back to your team on admission status — no black holes
- Bilingual (English / Spanish) intake and care teams
How a referral works
Three steps. We handle the rest.
Step 1
Call or email
Reach our intake nurse directly at (903) 470-1994 or care@azaleahospice.com. Give us the patient's name, location, and diagnosis — that's enough to start.
Step 2
We evaluate
An Azalea nurse evaluates the patient — at the bedside before discharge when timing is tight, or at home. We confirm eligibility and complete the Medicare election.
Step 3
We take over symptom care
Once admitted, the patient gets 24/7 on-call coverage, medications, equipment, and the full team. We keep your case managers updated on status.
